Job summary

Holcomb Behavioral Health System in Easton, PA seeks a Community-Based Case Manager to provide services to individuals eligible for behavioral health support. You will assess resources, carry a caseload, and coordinate care to help clients live safely in the community.

Responsibilities include developing strength-based service plans, crisis intervention, and on-call support 24/7. A PA driver's license or relevant degree/experience is required, with 2+ years in direct client contact preferred.

Qualifications

• Valid Pennsylvania driver’s license with no more than three points
• Bachelor’s degree in listed fields (or related coursework)
• High school diploma with 12 semester hours in sociology/social welfare/psychology/gerontology or other social science and 2 years of public/private human services experience with 2 years in direct client contact (1 year for Lehigh and Northampton counties)
• High school diploma and 5 years of mental health direct care experience in public/private human services with employment as a case management staff person prior to April 1, 1989.

Responsibilities

• Carry a caseload of up to 30 clients
• Perform case management duties for consumers in Behavioral Health Services
• Provide services based on Recovery Model principles
• Develop individualized, strength-based service plans that are measurable and lead to targeted outcomes
• Assist consumers in obtaining/maintaining basic needs such as housing, food, healthcare, employment, and socialization
• Assist consumers through assertive and creative efforts to gain needed resources and services identified in the service plan
• Monitor the consumer's participation in the recovery plan and support services
• Provide effective crisis assessment and crisis intervention to consumers when necessary
• Maintain professional relations with the consumer, their families, payers, community support service representatives, coworkers, and other agencies
• Ensure communication and coordination between all service providers and support systems
• Act as a single point of contact for multiple health and social services linkages
• Provide all authorized client service and supporting documentation for re-authorizations in a timely manner
• Submit accurate and timely payroll and billing documentation
• Be available for an on-call rotation 24/7

Education

Bachelor's degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education

High school diploma with 12 sem. hrs. in social sciences + 2 years in human services

High school diploma + 5 years mental health direct care experience
