Responsibilities

• Assess and evaluate adults to determine the need for case management services.
• Assist consumer entry into service and monitor services provided.
• Develop and monitor an Individual Treatment Plan (ITP) and ensure it is outcome-based.
• Coordinate treatment team meetings and ensure treatment plans are followed.
• Educate consumers about recovery markers and utilize progress measures.
• Follow agency policies and assist in enrollment in applicable health plans.
• Maintain caseload and meet billing expectations monthly, quarterly, and yearly.
