Job summary

Signify Health, Inc. is seeking physicians to conduct in-home health evaluations for health plan members, offering flexibility and a self-determined schedule.

You will oversee nurse practitioners or physician assistants contracted by Signify Health, with a monthly stipend based on activity and opportunities for additional pay-per-test earnings. The role requires licensure in North Carolina and residence in the state, with malpractice coverage during Signify Health assignments.

Qualifications

• Must be licensed in and living in North Carolina.
• Doctorate degree required for clinical practice.

Responsibilities

• Provide in-home health evaluations for health plan members, including medication and medical history review and physical assessment.
• Do not prescribe medications or order labs or alter current treatment plans; work with care team to support member health.

Physician collabs earn a monthly stipend based on the number of Signify Health-contracted nurse practitioners and/or physician assistants they oversee.

You must be

in order to qualify.

Interested in in-home visits as well?

Our clinicians provide personalized In-Home Health Evaluations to health plan members. These visits include a medication and medical history review, as well as a physical evaluation performed in a health plan member’s home. You will be provided a pre-populated, patient-specific form for each evaluation and do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.

Flexible schedule based on your preferences and availability

Additional compensation opportunities via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)
