Job summary

Signify Health, Inc. is seeking physicians to conduct in-home health evaluations for health plan members, with a flexible, self-determined schedule. You will oversee nurse practitioners and physician assistants contracted by Signify Health and perform comprehensive patient assessments in home settings.

Physicians must be licensed in North Carolina and reside there to qualify. A Doctorate is required, and while MD/DO licensure is preferred, the role emphasizes home visits without altering

Qualifications

• Licensed to practice in North Carolina.
• Doctorate of Medicine or Osteopathy required.
• Experience with in-home evaluations a plus.

Responsibilities

• Oversee Signify Health-contracted nurse practitioners and/or physician assistants.
• Conduct in-home health evaluations for health plan members.
• Provide patient histories and physical evaluations without altering current treatment regimens.

Physician collabs earn a monthly stipend based on the number of Signify Health-contracted nurse practitioners and/or physician assistants they oversee.

You must be

licensed in and living in North Carolina

in order to qualify.

Interested in in-home visits as well?

Our clinicians provide personalized In-Home Health Evaluations to health plan members. These visits include a medication and medical history review, as well as a physical evaluation performed in a health plan member’s home. You will be provided a pre-populated, patient-specific form for each evaluation and do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.

Flexible schedule based on your preferences and availability

Additional compensation opportunities via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)
