Your Health in Rock Hill, SC seeks a dedicated Cognitive Behavioral Specialist to support patients with dementia and cognitive impairments. You will work with an interdisciplinary team, visiting facilities and patients’ homes, and travel daily to designated service areas including Rock Hill.

The role emphasizes structured behavioral interventions, caregiver support, and coordination with medical treatment. Minimum 5 years in dementia care, with licensure in good standing preferred; certification

Qualificações

Minimum of five years of experience working with individuals with dementia or cognitive impairments.
Experience in geriatrics or memory care roles (Activities Director, CNA, MA, RN/LPN). Licenses must be in good standing if applicable.
Actively enrolled in, or willing to enroll in, a Dementia certification course.
Dementia Care certification preferred or willingness to pursue certification.
Familiarity with behavioral health medications and interventions for this population.
Ability to read and communicate effectively.
Strong written and verbal communication skills.
Basic computer knowledge.
Ability to demonstrate leadership and manage multiple responsibilities.
Good interpersonal and communication skills under all conditions.
Ability to foster a cooperative work environment.
Team player with sound judgment and flexibility.
Must be able to travel between offices and facilities; licensed driver with insured vehicle.

Responsabilidades

Manage a caseload of patients with cognitive behaviors.
Conduct patient visits with a cadence tailored to needs (weekly/biweekly/monthly).
Observe behavioral patterns and maintain detailed documentation.
Implement structured behavioral protocols and address triggers.
Monitor changes to adjust care plans and cognition.
Collaborate with primary care and specialists on treatment plans.
Participate in interdisciplinary case reviews and telehealth consults.
Educate and support caregivers on behavioral management.
Lead caregiver coaching sessions on behavior modification.
Lead family education initiatives on dementia and aging.
