Job summary

Oaks Integrated Care Inc. in Cherry Hill, NJ is seeking a Co-Occurring Specialist to join the PACT team. You will provide clinical assessments, service planning, wellness counseling, and case management to individuals with co-occurring disorders in a community-based setting.

The role focuses on intensive outpatient services, crisis intervention, and collaboration with the PACT team to reduce hospitalizations and support recovery and independence for program participants.

Qualifications

• Bachelor’s degree in a behavioral health field.
• CADC/LCADC credentials required.
• Valid driver’s license in good standing.

Responsibilities

• Provide clinical assessment, service planning, wellness counseling, and case management as required.
• Deliver services in individual and group modalities.
• Promote wellness and recovery through person-centered planning.
• Participate in ongoing monitoring of mental health status and needs.
• Provide crisis intervention and 24/7 on-call coverage on rotation.
