Job summary

The ACT/IDDT team delivers community-based supports to adults with severe mental illness, coordinating crisis and co-occurring services.

Requirements include a bachelor's or master's degree in social work, psychology, or counseling, eligibility for Michigan licensure, and certification as an Addictions Counselor or eligible within 3 years. EOE.

The Assertive Community Treatment (ACT)/lntegrated Dual Disorder (IDDT) Team is a multi-disciplinary, community based, treatment team providing a broad range of supports to individuals with severe and persistent mental illness.

The duties for this position include providing co-occurring, crisis, and case management support to adult consumers, their families and natural support groups. This individual will perform assessments, develop treatment plans, manage referrals/linkage, monitor progress, integrate treatment, coordinate benefits and maintain the consumer record.

MINIMUM QUALIFICATIONS:

• Bachelor’s degree or Master’s degree in social work, psychology, counseling from accredited program
• Eligible for licensure with the State of Michigan
• Certified as an Addictions Counselor or eligible to acquire certification within 3 years

DESIREABLE QUALIFICATION:

• One year experience with mental health services
