Job description

Role Overview

As a hospice Certified Nursing Assistant (CNA), you will be a member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and promote comfort. The hospice aide observes the patient, reports observations, documents care, and collaborates with the case manager to ensure quality, continuity, and safety of patient care.

Key Responsibilities
  • Perform personal care needs after being assigned a patient and attend to the patient's requests promptly.
  • Report any task that cannot be performed to the Case Manager immediately.
  • Provide patient and family with positive communication within ethical and respectful care, maintaining confidentiality.
  • Meet safety needs of the patient and use equipment safely and properly (foot stools, side rails, oxygen, etc.).
  • Give personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.
  • Assist in dressing and undressing patients as assigned.
  • Plan and prepare nutritious meals, including shopping, as assigned.
  • Assist in feeding the patient as assigned.
  • Take and record oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate demonstrated competency.
  • Provide proper care and observation of patient's skin to prevent breakdown over bony prominences.
  • Assess and report on patient's condition and significant changes to the Case Manager.
  • Maintain awareness of caregiver or other individuals living with the patient and interpersonal issues.
  • Assist in ambulation and exercise as instructed by the hospice nurse or therapist.
  • Provide normal range of motion and transfers/positioning as directed.
  • Assist with self‑administered medications as allowed by state regulations, such as reminders.
  • Offer and assist with bedpans and urinals.
  • Assist with light laundry needs as assigned.
  • Perform range of motion and simple procedures as part of an extensional therapy service as ordered, with demonstrated competency.
  • Provide respite for patient’s/family/caregiver when on‑site, as appropriate.
  • Keep patient's living area clean and orderly as assigned.
  • Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
  • Participate in quality assessment and performance improvement teams and activities.
  • Respect patient and family/caregiver environment and patient's personal needs.
Qualifications
  • Meet state training requirements for a CNA.
  • Completed a minimum of 75 hours of classroom and supervised practical training (16 hours of classroom before 16 hours of practical training).
  • Be at least 18 years of age.
  • Be able to read and follow written instructions and document care provided.
  • Understand hospice philosophy and be comfortable providing specialized care to the terminally ill.
  • Provide satisfactory references from previous employers.
  • Be self‑directing and able to work with little direct supervision.
  • Comfortable dealing with death and dying.
  • Maintain a calm manner when in a patient’s home.
  • Demonstrate empathy for the needs of the ill, injured, frail and impaired.
Additional Requirements
  • Possess and maintain current CPR certification.
  • Demonstrate tact, patience and good personal hygiene.
  • Hold a valid driver's license with a vehicle that is insured per organization requirements and in good working order.
  • Successfully complete a probationary period documented by supervisor and employee to ensure competency in the above qualifications.
Job Limitations

The hospice aide will not engage in nursing practice as defined by the Nurse Practice Act. The hospice aide will not administer medications, take physician orders, or perform procedures requiring licensed nurse training such as sterile techniques.
