Job summary

Otterbein Hospice, part of Otterbein SeniorLife, is hiring a CNA/STNA to support hospice patients across Ohio. This role focuses on compassionate, person-centered care and works with an interdisciplinary team.

We require active Ohio STNA registry, CPR certification, and at least one year of direct patient care. Travel is required to cover Loveland, Maineville, Lebanon, Batavia and surrounding areas.

Qualifications

• Must have completed at least the 9th grade; HS diploma or GED preferred.
• CNA/STNA certification must be active on the Ohio STNA Registry; CPR certification required.
• Minimum 1 year of direct patient care in institutional or home care setting; hospice experience preferred.
• Ability to work within a multidisciplinary team; understanding of Hospice philosophy.
• Valid driver’s license and reliable insured transportation.

Responsibilities

• Provide personal care to patients.
• Respect patient rights and maintain dignity for patients and families.
• Observe, record, and report patient care, symptoms, and status to RN.
• Assist in feeding patients as needed.
• Attend required in-services (12 hours per year) and IDT meetings.
• Travel to see patients in Toledo/Perrysburg areas.

Responsibilities

• Provides personal care
• With respect to patient rights, ensures all care is provided with respect and dignity to patients and their families
• Demonstrates and maintains a positive, professional manner
• Keeps a record of hospice CNA/STNA plan of care for each patient assigned
• Observes, records, and reports the patient’s care, symptoms, emotional and physical status
• Watches, observes, and reports any physical and gross behavioral changes in the patient’s condition or family situation to the RN
• Assists in feeding the patient as needed
• Communicates appropriately with the patient/family and agency personnel
• Attends required in-services and meetings as identified by the agency (must have twelve (12) hours of in-services a year).
• Attends and participates in regularly scheduled Hospice interdisciplinary team (IDT) meetings to coordinate plan of care, follow up on changes and other communication, problem-solve, receive staff support and in-service education
• Participates in Quality Assurance/Performance Improvement (QAPI) activities, as indicated
• Will need to travel to see patients in the Toledo/Perrysburg areas

Qualifications

• Minimum age requirement of 18.
• Education: Must have completed the 9th grade, with ability to read or write at an adequate level. High school graduates and GED preferred.
• Certification: CNA/State Tested Nursing Assistant (STNA) certification - Must be on the State Tested Registry in Ohio and remain active, CPR required.
• Experience:
• Minimum of one (1) year of direct patient care experience in an institutional or home care setting required with community health/hospice experience preferred.
• Must demonstrate the ability to be able to work within a multidisciplinary team setting.
• Understanding of Hospice philosophy and needs of terminally ill.
• Valid driver’s license and reliable transportation that is insured in accordance with Ohio requirements.
