Qualifications

• Current BLS/CPR certification (as required by SNFs/ALFs)

Responsibilities

• Organizes transportation of residents to medical appointments based on priority.
• Transports residents to and from inpatient/outpatient programs and facilities.
• Develops driving schedule around residents' appointments and provides transport after hospital discharge.
• Reports changes in resident status during transport to the clinical supervisor.
• Transports medical records for signatures to medical offices.
• Maintains safety and vehicle condition, logs mileage, and keeps van clean.
• Assists clinical staff with safe lifting and transfers as directed.
