Job summary

Sanctuary Hospice is seeking a Per Diem Hospice Aide (CNA) to provide compassionate, hands-on care to patients at the end of life. You will work with our nursing team to meet physical, emotional, and personal needs with dignity and empathy.

Responsibilities include personal care, family support, and accurate documentation, while adhering to the plan of care and agency standards. Requires CNA credential, CPR if required, and reliable transportation.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered CNA certificate.
• Minimum one year CNA experience in acute or long-term care; hospice preferred.
• CPR certification if state-mandated.
• Current driver's license, vehicle insurance, and reliable transportation.

• Provide Personalized Care: follow the plan of care to ensure patient comfort and wellbeing.
• Assist with Personal Care: help with bathing, oral hygiene, bedding, clothing, and shampooing.
• Be Dedicated to Customer Service: deliver exceptional care and support.
• Provide Support for Families: guide and assist families during hospice care.
• Maintain Accurate Documentation: document care details per policies and regulations.

Make Every Moment Matter as a Hospice Aide (CNA)! Are you a caring and dedicated Certified Nursing Assistant looking to make a meaningful impact? Sanctuary Hospice is seeking a Per Diem Hospice Aide (CNA) to join our compassionate team, providing hands-on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

Per Diem, As Needed 7 am - 3:30 pm

McHenry & Lake County, Cities Included but Not Limited to Crystal Lake, McHenry, Algonquin, Lake Zurich and Fox Lake

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well-being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state-mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

$21.00 - $23.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
