Job summary

Crescent Hospice is seeking a compassionate Certified Nursing Assistant to join our per diem hospice team in Conway, SC. You will work closely with our nursing team to provide hands-on patient care at the end of life, ensuring comfort, dignity, and respect in the home setting.

Requirements include a high school diploma or GED, active CNA license in the state, 1 year of CNA experience, CPR certification, and reliable transportation.

Qualifications

• High School Diploma or GED required.
• Active, unencumbered CNA license in the state.
• Minimum 1 year CNA experience in acute or long-term care; hospice experience a plus.
• CPR certification if state-mandated.
• Current driver’s license and access to dependable transportation.

Responsibilities

• Provide personalized care per care plan and promptly attend to patient and family requests.
• Assist with personal care including bathing, hygiene, bedding, clothing, and grooming.
• Deliver high-quality customer service and support to patients and families.
• Provide guidance to families and support them in caring for terminally ill loved ones.
• Maintain accurate documentation in patient records and agency systems.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well-being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state-mandated
• Current driver’s license, vehicle insurance, and access to dependable transportation.

$17.00 - $19.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
