Qualifications

• High school graduate or GED preferred, but not required.
• Minimum age of 16.
• Prior clinical or nursing assistant experience in long-term care preferred.
• CNA certification required with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement.
• CNA training and certification required and provided by facility.
• Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year.
• A minimum of 12 hours continuing education are required annually.

Responsibilities

• Provides assistance with basic health care needs including daily living activities such as bathing, toileting, grooming, dressing, recording vital signs, and providing psychosocial support.
• Assists residents with transferring, repositioning, and walking using transfer techniques and equipment and provides range of motion and passive exercises.
• Completes tasks related to food preparation and dining, creating a positive dining experience for residents.
• Assists residents with meals and snacks, provides water and supplements as appropriate.
• Aids providers and nursing staff with procedures when needed.
• Documents resident interactions and outcomes related to care, behavior, activity, and dietary.
• Communicates changes in resident condition to the charge nurse.
• Collects non-invasive body fluid specimens and gathers vital signs.
• Provides housekeeping and laundry services to keep living areas clean.
• Assists with wellness and recreational programs and encourages participation.
