Job summary

JourneyCare Hospice in Chicago, IL is seeking compassionate Home Health Aides / CNAs to join our growing team. This full-time role offers competitive pay and a clear path for growth, with immediate opening and comprehensive benefits.

As a CNA, you will provide personal care, assist with activities of daily living, plan and prepare meals, and monitor patients, reporting changes to the Case Manager. Valid state CNA certification and a clean registry are required.

Qualifications

• State-certified nurse aide is required.
• Must be listed on the state aide registry with a clear record.
• Must be at least 18 years old (per state law).
• Valid driver’s license with insured vehicle per state requirements.

Responsibilities

• Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linens.
• Assist in dressing and undressing patients.
• Plan and prepare nutritious meals, including shopping.
• Assist in feeding the patient.
• Monitor and provide proper care to prevent skin breakdown and injuries.
• Report patient condition and significant changes to the Case Manager.

Monday-Friday 8:30AM-5PM - NO WEEKENDS - NO HOLIDAYS

What You’ll Do

• Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linens
• Assisting in dressing and undressing patients
• Planning and preparing nutritious meals, including shopping
• Assisting in feeding the patient
• Providing proper care and observation of patient’s skin to prevent breakdown of tissue
• Reporting on patient’s condition and significant changes to the Case Manager.
