As part of an interdisciplinary private duty team, our CNA's (Home Health Aides) provide patients with care in the comfort of their own home in a one-on-one setting. We serve patients throughout Milwaukee, Waukesha, Ozaukee, and Washington Counties (mileage reimbursed between visits).

As a Private Duty Home Health Aide, you will provide direct assistance with activities of personal care, maintenance of patient health, agency facilitation of treatment, and contribute to a healthy environment for patients. These activities are provided under the supervision of a Registered Nurse or Therapist, in accordance with physicians' orders.

  1. Support patient care by reviewing the care plan in the home prior to providing cares. Notify the Case Manager of issues or concerns regarding the care plan and when cares need revision.
  2. Assist with daily living activities such as bathing, dressing, transfers, range of motion exercises, toileting, etc., under the supervision of the Case Manager.
  3. Explain to patients the care being provided and involve them in cares.
  4. Complete mobile care tasks upon arrival and departure from patients' homes per agency policy. Mobile care includes tasks in accordance with the care plan. Ensure mileage logs are completed accurately and timely.
  5. Work cooperatively with patients, care providers, family members, and agency staff by responding to patient needs, communicating effectively, and reporting pertinent information to the Case Manager promptly.
  6. Perform home maintenance tasks as per the care plan, including house cleaning, washing dishes, laundry, meal preparation, transportation, etc.

Minimum Requirements:
  • Wisconsin CNA certification for work in federally certified home health agencies and hospices.
  • Prior experience working with the public is required.
  • Valid Wisconsin Driver's License, reliable and insured vehicle.
Additional Details:

Night shift, 8:00 pm - 8:00 am, including weekends and holidays. Hourly, non-exempt. Position is private duty.
