As part of an interdisciplinary private duty team, our CNA's (Home Health Aides) provide patients with care in the comfort of their own home in a one-on-one setting. We serve patients throughout Milwaukee, Waukesha, Ozaukee, and Washington Counties (mileage reimbursed between visits).

Position: Private Duty Home Health Aide

You will provide direct assistance with activities of personal care, maintenance of patient health, agency facilitation of treatment, and contribute to a healthy environment for patients. All activities are performed under the supervision of a Registered Nurse or Therapist, following physicians' orders.

  1. Review care plans in the home prior to providing care, and notify the Case Manager of issues or concerns requiring care plan revisions.
  2. Assist with daily living activities such as bathing, dressing, transfers, range of motion exercises, toileting, etc., under the supervision of the Case Manager.
  3. Explain the care being provided to the patient and involve them in cares.
  4. Complete mobile care tasks upon arrival and departure from patients' homes as per agency policy, including accurate mileage logs.
  5. Collaborate effectively with patients, care providers, family members, and agency staff by responding to patient needs and reporting pertinent information to the Case Manager promptly.
  6. Perform home maintenance tasks as per the care plan, including house cleaning, washing dishes, laundry, meal preparation, and transportation.

Minimum Requirements:
  • Wisconsin CNA certification for work in federally certified home health agencies and hospices
  • Prior experience working with the public
  • Current/valid WI driver's license, reliable and insured vehicle
Additional Details:

Night shift, 8:00 pm - 8:00 am, including weekends and holidays. Hourly, non-exempt position. Private duty role.
