About the Role

The future of health care is home care. As part of an interdisciplinary private duty team, our CNA's (Home Health Aides) provide patients with care in the comfort of their own home in a one-on-one setting. We serve patients throughout Milwaukee, Waukesha, Ozaukee, and Washington Counties (mileage reimbursed between visits).

Responsibilities

1. Support patient care by reviewing care plan in the home prior to providing cares. Notify Case Manager with issues / concerns regarding care plan and when cares need revision.
2. Provide assistance with activities of daily living under the supervision of the Case Manager, such as bathing, dressing, transfers, range of motion exercises, toileting, etc.
3. Explain to patients the care being provided and involve them in cares.
4. Complete mobile care tasks upon arrival and departure from patients' homes per Agency policy, including timely and accurate mileage logs.
5. Work cooperatively with patients, care providers, family members, and Agency staff by listening, responding to needs, communicating effectively, and reporting pertinent information promptly.
6. Perform home maintenance tasks as per the plan of care, including house cleaning, laundry, meal preparation, and transportation.

Qualifications

• Wisconsin CNA certification for work in federally certified home health agencies and hospices
• Prior experience working with the public
• Valid Wisconsin driver's license, reliable and insured vehicle

Additional Details

Night shift, 8:00 pm - 8:00 am, including weekends and holidays. Hourly, non-exempt position. Private duty role.
