As part of an interdisciplinary private duty team, our CNA's (Home Health Aides) provide patients with care in the comfort of their own home in a one-on-one setting. We serve patients throughout Milwaukee, Waukesha, Ozaukee, and Washington Counties (mileage reimbursed between visits).

Position: Private Duty Home Health Aide

You will provide direct assistance with activities of personal care, maintenance of patient health, agency facilitation of treatment, and contribute to a healthy environment for patients. These activities are provided under the supervision of a Registered Nurse or Therapist, in accordance with physicians' orders.

  1. Support patient care by reviewing care plans in the home prior to providing cares. Notify the Case Manager with issues or concerns regarding the care plan and when cares need revision.
  2. Assist with daily living activities such as bathing, dressing, transfers, range of motion, toileting, etc., under the supervision of the Case Manager.
  3. Explain the care being provided to patients and involve them in cares.
  4. Complete mobile care tasks upon arrival and departure from patients' homes per agency policy, including tasks in accordance with the care plan. Maintain accurate mileage logs.
  5. Work cooperatively with patients, care providers, family members, and agency staff by listening, responding to needs, engaging in communication, and reporting pertinent information to the Case Manager promptly.
  6. Perform home maintenance tasks as per the plan of care, including house cleaning, washing dishes, laundry, meal preparation, and transportation.

  • Wisconsin CNA certification for work in federally certified home health agencies and hospices
  • Prior experience working with the public
  • Current/valid Wisconsin Driver's License and reliable, insured vehicle
Additional Details:
  • Night shift, 8:00 pm - 8:00 am, including weekends and holidays
  • Hourly, non-exempt position
