As part of an interdisciplinary private duty team, our CNA's (Home Health Aides) provide patients with care in the comfort of their own home in a one-on-one setting. We serve patients throughout Milwaukee, Waukesha, Ozaukee, and Washington Counties (mileage reimbursed between visits).

Position: Private Duty Home Health Aide

You will provide direct assistance with activities of personal care, maintenance of patient health, agency facilitation of treatment, and contribute to a healthy environment for patients. These activities are performed under the supervision of a Registered Nurse or Therapist, in accordance with physicians' orders.

1. Support patient care by reviewing care plans prior to providing cares. Notify the Case Manager of issues or concerns regarding the care plan or when cares need revision.
2. Assist with daily living activities such as bathing, dressing, transfers, range of motion exercises, toileting, etc., under the supervision of the Case Manager.
3. Explain the care being provided to patients and involve them in their cares.
4. Complete mobile care tasks upon arrival and departure from patients' homes per agency policy, including accurate mileage logs.
5. Work cooperatively with patients, their families, care providers, and agency staff by responding to patient needs and communicating pertinent information to the Case Manager promptly.
6. Perform home maintenance tasks as per the care plan, including house cleaning, laundry, meal preparation, and transportation.

• Valid Wisconsin Driver's License and reliable, insured vehicle

Additional Details:

• Night shift, 8:00 pm - 8:00 am, including weekends and holidays
• Hourly, non-exempt position
