4 YOUR PEACE OF MIND LLC in Hartford, CT is seeking a Home Health Aide to support clients in daily living tasks as outlined in a care plan created by the RN or Supervisor of Clinical Services. HHAs are supervised and scheduled by the office to ensure consistent care.

The role requires completion of HHA training or CNA certification and competency evaluation, with the ability to lift, transfer, and assist with mobility.

Qualifikationen

• Must be at least 18 years old.
• Minimum one year of verifiable personal care experience (3 references required).
• Completion of HHA training or CNA certification, plus competency evaluation.
• Physically able to perform duties (lifting, transfers, ambulation).
• Must read, write, and speak English fluently.
• Reliable transportation to and from assignments.

Aufgaben

• Follow care instructions from an RN with care plan updates as needed.
• Assist with personal grooming, mobility, transfers, and exercises.
• Provide nutrition support, including feeding and hydration.
• Assist with toileting and elimination.
• Light housekeeping necessary for client safety.
• Medication assistance (self-administered only): retrieve container, confirm prescription, remind to take, observe self-administration.
• Other activities as taught and delegated by an RN per Connecticut law and agency policy.
