Fallon Health is hiring Health Aides and CNAs for Summit ElderCare in Massachusetts. The role supports ADLs at the PACE center and in homes, with a focus on safe, compassionate care and adherence to CDC guidelines. Sign-on benefits and ongoing training are provided.

We seek individuals with HHA/CNA training, a valid license, and at least one year of relevant experience. A driver’s license and reliable transportation are required to support community-based visits.

Qualifications

• Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but equivalent work experience is acceptable.
• License/Certifications: Certification as a Home Health Aide or Nursing Assistant is desired.
• Driving a motor vehicle is an essential job function; must possess a valid driver’s license and maintain required insurance.
• One year of experience with frail/older adults, preferably in a PACE program; Summit Eldercare training may be required.
• Must complete Summit Eldercare competencies annually.
• Massachusetts Wage Transparency Act pay range: $21.50–$24.00 per hour; final compensation depends on experience.

Responsabilités

• Assist with Activities of Daily Living (ADL) needs at the PACE center and in participants’ homes.
• Contribute to care plan development with the Interdisciplinary Team; assure continuity of care.
• Carry out non-skilled treatments: vital signs, transfers, toileting, bathing, dressing.
• Support activity staff with individual and group programs and evaluate program effectiveness.
• Assist therapists with ordered treatments and participant-specific activities.
• Maintain a clean and safe environment at the PACE center and participants’ homes.
• Assist with meal and snack preparation, serving, and cleanup.
• Document accurately in the EMR and other systems as required.
• Participate in routine cleaning and disinfection per CDC guidelines.
• Help distribute work among health aides and participate in team meetings.
