Fallon Health, Dartmouth, MA, is seeking a CNA/Home Health Aide for per diem roles with pay between $20.00 and $24.00 per hour. The role involves personal care, light housekeeping, and ADL support within participant homes and the PACE center.

Under RN supervision, you will contribute to care plans, maintain infection control, and document in the EMR, while upholding Summit ElderCare policies.

• Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable; equivalent work experience may be accepted.
• Certification as a Home Health Aide or Nursing Assistant is desired. Reliable transportation is required.
• One year of experience working with frail and/or older adults, preferably in a PACE program.
• Must complete a standardized set of Summit ElderCare competencies for Health Aides annually.

Responsabilidades

Summary

Provides personal care, light housekeeping, and assistance with activities of daily living (ADLs) as outlined in each SE participant’s plan of care in participant homes and at the PACE center; exercises independent judgment; reports changes in participant status to the IDT; carries out infection control precautions and increased cleaning and disinfecting of the PACE center in accordance with CDC guidelines.

Responsibilities

• Under the supervision of the RN, assists with the ADL needs of participants both at the PACE center and in participants' homes.
• Contributes to the development of a care plan for participants through interaction with other members of the Interdisciplinary Team.
• Collaborates with team members to ensure appropriateness and continuity of care.
• Carries out non‑skilled treatments including vital signs, transfers, toileting, bathing, dressing.
• Assists supervisors and activities staff with individual and group programs, planning treatments, motivating participation, and evaluating program effectiveness.
• Assists registered therapists and therapy assistants with ordered treatments and activities for each participant.
• Maintains a clean and safe environment in the PACE center and participants' homes.
• Assists with meal and snack preparation, serving, feeding, and clean‑up.
• Uses safe techniques in all interactions with participants.
• Provides accurate and timely documentation in the EMR and other systems as required.
• Participates in a schedule of daily cleaning and disinfecting of the PACE center per CDC guidelines.
• Distributes work among health aides to ensure care needs and site tasks are completed timely and appropriately.
• Attends regular team meetings for health aides at the respective site.
• Performs all duties in accordance with FH and Summit ElderCare policies and procedures.

Qualifications

Education: Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable; equivalent work experience may be accepted in lieu of a certificate.

Certification: Certification as a Home Health Aide or Nursing Assistant is desired. Reliable transportation is required.

Experience: One year of experience working with frail and/or older adults, preferably in a PACE program. If under one year, must complete the Summit ElderCare training program before working independently with participants.

Requirements: Must complete a standardized set of Summit ElderCare competencies for Health Aides annually.

Pay Range Disclosure: In accordance with the Massachusetts Wage Transparency Act, the pay range is $20.00 to $24.00 per hour, which reflects the expected pay at the time of posting. Final compensation will depend on the candidate’s experience, skills, and fit with the role’s responsibilities.
