Job summary

Qualifications

• Completion of an approved training program or equivalent work experience.
• Certification as a Home Health Aide or Nursing Assistant is desirable.
• One year of experience with frail or older adult populations preferred.

Responsibilities

• Assist with ADL needs at the PACE center and participants’ homes.
• Contribute to care plans through team interaction.
• Collaborate for care continuity.
