Logan Health in Kalispell, MT is seeking a compassionate Medical Social Worker to support hospice and home health patients and families across Flathead and Lake counties. You will provide psychosocial assessments, case management, and grief support as part of an interdisciplinary team.

A CSWE‑accredited MSW with at least one year of healthcare social work experience is required. The role includes resource coordination, advance directives review, discharge planning, and travel within the service

Competenze

• MSW from CSWE‑accredited program required.
• Minimum of one year of healthcare social work experience.
• Prior grief or bereavement support experience desired.
• Hospice or home health experience preferred.
• Ability to independently manage a patient caseload and respond to urgent situations.
• Valid driver's license and reliable transportation.

Mansioni

• Complete psychosocial assessments for hospice and home health patients within timeframes.
• Provide ongoing case management and adjust visit frequency as needs change.
• Deliver anticipatory grief support to patients and families.
• Facilitate a weekly grief support group.
• Coordinate with community programs and resources for patients and families.
