Job summary

Senior Community Care of Colorado PACE (Montrose) is seeking a CNA to provide daily living assistance in a community-based care setting. You will support activities of daily living, monitor health, and contribute to a safe, compassionate home environment for participants.

This full-time on-site role requires a CO CNA license, a year of experience with the elderly, a valid driver’s license, and willingness to travel to participant homes as needed.

Qualifications

• High School diploma or equivalent is required.
• Colorado CNA license is required.
• At least 1 year experience with frail/elderly population.
• Must have valid driver’s license, proof of insurance, and transportation.

Responsibilities

• Assist participants with daily living activities and health monitoring.
• Provide supportive care and work with Interdisciplinary Team; travel to participant homes.

Pay Range: $19.50 - $23.50 per hour (based previous work experience)

Schedules: Schedule: Full-Time|80 hours per pay period | Days and shift times will vary and includes occasional weekends and holiday rotation

Travel: Requires travel to participant homes. *You must be at least 21 years of age!

What You Bring (Requirements)

• High School diploma or equivalent required.
• Current License as Certified Nursing Assistant (C.N.A) in the state of Colorado require.
• A minimum of one year’s experience in working with the frail or elderly population required.
• Must have a valid driver’s license, proof of insurance and have means of transportation.

What You’ll Be Responsible For

The Certified Nursing Assistant is responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to the preservation of assisting participant’s in living independently in the community. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior CommUnity Care. Functions as a member of the Interdisciplinary Team. This position may require travel to participant homes.

• Assists participants with Independent Activities of Daily Living (IADL’s) including but not limited to grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care as assigned. Reports changes in participant’s condition and needs to the Personal Care Supervisor (or designee).
• Assists with participant activities in the Day Center, including meal service and clean-up.
