Job summary

Safe Life Home Health Care, LLC in Kane County, IL, is seeking dedicated CNAs to join our team and provide compassionate in-home care. As a Home Health Aide/CNA, you will assist patients with daily activities, monitor health, and support their comfort and safety.

This role offers flexible scheduling and the opportunity to work closely with a caring medical team. Candidates should have an active CNA certificate, CPR, a valid driver's license, and good communication skills.

Qualifications

• Active CNA certificate required.
• CPR certification required.
• Valid driver's license and car insurance.
• Prior home health experience is a plus.
• Familiarity with EMR Kinnser by WellSky is advantageous.
• Excellent communication skills are essential.
• Compassion and kindness are essential qualities.

Responsibilities

• Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming and toileting.
• Support mobility and safe transfers.
• Monitor and record vital signs.
• Provide medication reminders and ensure schedule adherence.
• Assist with meals and monitor dietary needs.
• Maintain patient hygiene and comfort and change linens as needed.
• Observe and report changes to the nursing team.
• Document care provided and vital signs in the EMR system (Kinnser by WellSky).
• Communicate clearly with patients, families, and the healthcare team.
• Follow infection control and safety protocols to maintain a safe environment.
• Provide transportation to medical appointments as needed.
• Offer emotional support and companionship to patients.
