Job summary

Safe Life Home Health Care, LLC is seeking a compassionate Home Health Aide / CNA to join our Chicago team. You will assist patients with daily activities, monitor vital signs, and support their comfort and safety in home care settings.

Responsibilities include ADLs, mobility support, medication reminders, EMR documentation in Kinnser by WellSky, and collaborating with nurses and therapists. Flexible hours and education reimbursement offered.

Qualifications

Active CNA certificate.
Valid driver's license and car insurance.
CPR certification.
Prior home health experience is preferred.
Familiarity with EMR Kinnser by WellSky is advantageous.
Excellent communication skills.
Compassion and kindness are essential.

Responsibilities

Assist with Activities of Daily Living (ADLs) including bathing, dressing, grooming and toileting.
Support mobility: walking, transferring, and use of mobility aids.
Monitor and record vital signs (BP, pulse, temperature).
Provide medication reminders and ensure adherence to schedules.
Assist with meals and monitor dietary restrictions.
Maintain patient hygiene and comfort, including linens and oral care.
Document care provided and vital signs in the EMR (Kinnser by WellSky).
Communicate clearly with patient, family, and healthcare team.
Follow infection control and safety protocols.
Provide transportation to appointments as needed.
Offer emotional support and companionship.
Collaborate with nurses and therapists for comprehensive care.
Educate patients and families on self-care and safety.
