Qualifications

• Active CNA certificate and CPR certification are required.
• Driver's license and car insurance are required.
• Excellent communication skills and compassionate care are essential.

Responsibilities

• Assist with ADLs including bathing, dressing, grooming, and toileting.
• Support mobility and transfers; assist with walking and use of mobility aids.
• Take and record vital signs (BP, pulse, temperature).
• Provide medication reminders and ensure adherence to schedules.
• Assist with meal preparation and monitor dietary restrictions.
• Maintain patient hygiene and comfort; change linens and provide oral care.
• Observe and report changes in patient condition to nursing staff.
• Document care, vital signs, and changes in EMR (Kinnser by WellSky).
• Communicate clearly with patient, family, and healthcare team.
• Follow infection control and safety protocols; assist with transportation as needed.
• Provide emotional support and companionship; collaborate with nurses and therapists.

• Vital Sign Monitoring: Take and record vital signs, including blood pressure, pulse, and temperature.
• Medication Reminders: Assist patients with medication reminders and ensure compliance with medication schedules.
• Nutrition Assistance: Help with meal preparation, feeding, and monitoring dietary restrictions as prescribed.
• Hygiene and Comfort: Maintain patient hygiene and comfort, including changing bed linens and providing oral care.

Qualification

• Active CNA certificate
• Valid driver's license and Car insurance
• CPR certification
• Prior experience in home health is a plus.
