Job summary

Safe Life Home Health Care in Chicago, IL is seeking dedicated CNAs to join our team and provide compassionate in-home care. You will assist patients with daily activities, monitor vital signs, remind medications, and support their overall well-being.

Qualifications

• Active CNA certificate.
• CPR certification.
• Valid driver\'s license and car insurance.
• Prior experience in home health is a plus.
• Familiarity with EMR Kinnser by WellSky is advantageous.
• Excellent communication skills.

Responsibilities

• Assist with Activities of Daily Living (ADLs) for home health patients.
• Mobility support: assist with walking, transfers, and mobility aids.
• Vital sign monitoring and recording.
• Medication reminders and adherence support.
• Nutrition assistance with meal prep and monitoring dietary restrictions.
• Hygiene and comfort, including changing bed linens and oral care.
• Observation and reporting of changes to nursing team.
• Accurate documentation in EMR (Kinnser by WellSky).
• Effective communication with patient, family, and healthcare team.
• Maintain a safe environment following infection control protocols.
• Provide transportation to medical appointments and assist with mobility.
• Offer emotional support and companionship.
• Collaborate with nurses and therapists to provide comprehensive care.
• Educate patients and families on self-care and safety.
• Show compassion and empathy in care delivery.
