Safe Life Home Health Care in Kane County, IL seeks dedicated CNAs to join our team as Home Health Aides. You will provide compassionate care to home health patients, assist with daily activities, and support their overall well-being.

Responsibilities include ADLs, vital signs, medication reminders, nutrition support, mobility, and documentation in EMR (Kinnser by WellSky). Collaboration with nurses and therapists ensures safe, personalized care.

Qualifications

Active CNA certificate and CPR certification are required.
Driver's license and car insurance are required.
Excellent communication skills and compassionate care are essential.

Responsibilities

Assist with ADLs including bathing, dressing, grooming, and toileting.
Support mobility and transfers; assist with walking and use of mobility aids.
Take and record vital signs (BP, pulse, temperature).
Provide medication reminders and ensure adherence to schedules.
Assist with meal preparation and monitor dietary restrictions.
Maintain patient hygiene and comfort; change linens and provide oral care.
Observe and report changes in patient condition to nursing staff.
Document care, vital signs, and changes in EMR (Kinnser by WellSky).
Communicate clearly with patient, family, and healthcare team.
Follow infection control and safety protocols; assist with transportation as needed.
Provide emotional support and companionship; collaborate with nurses and therapists.
