Job summary

Suncoast Hospice, a member of Empath Health, seeks a caring CNA to provide personal care and daily living assistance for patients in the Seminole/Largo area, with office in Palm Harbor. You will report changes to the RN and document care activities, supporting families through the care journey.

The role requires a Florida CNA license, a valid Florida driver’s license, and 1–3 months of related experience or training. Travel to patient sites is expected with reliable transportation and insurance.

Qualifications

• One to three months related experience and/or training preferred.
• Valid Florida Board of Nursing certification (CNA License).
• Valid Florida State Driver’s License.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living.
• Assist patients with taking medications that are ordinarily self-administered and pre-poured as designated in the Plan of Care.
• Report changes in patient’s medical, nursing, rehabilitative, and social needs to the registered nurse related to the plan of care and document the changes.
• Collaborate with the primary RN/Counselor and participate in closure contact with family/caregivers after a patient’s death.
