Job summary

Suncoast Hospice, a member of Empath Health, is seeking a caring CNA to provide personal care and activities of daily living for patients in the Seminole/Largo area, with the office in Palm Harbor. Since 1977, Suncoast Hospice has served Pinellas and Hillsborough counties with compassionate end-of-life care.

Qualifications

• One to three months related experience and/or training preferred.
• Valid Florida Board of Nursing certification (CNA License).
• Valid Florida State Driver’s License.
• Reliable transportation to perform tasks timely and appropriately.
• Proof of valid automobile insurance.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living.
• Assist patients with taking medications that are ordinarily self-administered and pre-poured as designated in the Plan of Care.
• Report changes in the patient’s medical, nursing, rehabilitative, and social needs to the RN per the plan of care.
• Document changes and to whom they were reported.
• Collaborate with the primary RN/Counselor after death and participate in family closure as needed.

Job description

Suncoast Hospice, a member of Empath Health, is seeking a caring and motivated CNA to provide personal care and tasks related to the activities of daily living for patients in facilities in the Seminole/Largo area, the office is located in Palm Harbor. Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way.

What you'll Do

• The provision of hands-on personal care and assistance with activities of daily living.
• Assisting patients with taking medications that are ordinarily self-administered and have been pre-poured as designated in the Plan of Care.
• Hospice Aides must report changes in the patient's medical, nursing, rehabilitative, and social needs to a registered nurse, as the changes relate to the plan of care.
• Hospice Aide must document the changes and to whom they were reported to.
• Following a patient's death collaborates with primary RN/Counselor and participates as needed in closure contact with family/caregivers.
