Trinity Health PACE in West Springfield, MA is seeking a Day Center Aide (CNA) for a full-time day shift. You will provide personal care, monitor health status, and support daily living activities for frail or elderly participants.

You will work with an interdisciplinary team, document changes, escort to appointments, and maintain safety protocols. A high school diploma and an active MA CNA license are required, with at least one year of relevant experience; training may be provided if needed.

Qualifikationen

• Active Certified Nursing Assistant (CNA) in MA required.
• Experience with frail/elderly populations preferred.
• Must be able to perform personal care tasks and mobility assistance.
• Must meet physical requirements for lifting and mobility.

Aufgaben

• Participate as a member of the interdisciplinary team assessing participants and reporting changes.
• Provide personal care assistance for activities of daily living and mobility support.
• Deliver nursing care as assigned within PACE policies and safety guidelines.
• Ensure participant safety and respond to emergencies and safety hazards.
• Provide behavioral supervision and emotional reassurance as needed.
• Assist with meals, recreation, and light housekeeping.
• Identify signs of physical, emotional, or behavioral changes and report.
• Adhere to infection control, PPE, and hazardous materials handling policies.
• Escort participants to appointments and assist with transportation as needed.
