Job summary

PACE Organization of Rhode Island seeks a compassionate Certified Nursing Assistant to provide personal care and support to participants at the Day Center. You will follow individualized care plans, monitor health signs, assist with activities of daily living, and document care activities in the electronic health record with accuracy.

This role requires 1 year CNA experience preferred, ability to lift up to 25 pounds, CPR certification, and a valid Rhode Island CNA license.

Qualifications

• Ability to maintain confidentiality and privacy of participant information.
• Dependable and punctual with consistent attendance.
• Ability to read, write and comprehend English.
• Ability to perform clinical/skilled tasks within scope of position.
• Proficiency with MS Office Suite or similar software.

Responsibilities

• Provide personal care including toileting, bathing, dressing and grooming.
• Monitor vital signs and assist with weight monitoring as directed.
• Follow individual care plans and participate in care plan meetings.
• Maintain accurate logs and documentation of care activities.
• Assist with meals, housekeeping, and transportation as needed.
• Support restorative activities and supervise participants as required.
• Ensure safety and hygiene in care areas and utilize PPE as required.
