Job summary

Trinity Health PACE in Massachusetts seeks a Day Center Aide to provide personal care and health monitoring for participants as part of an interdisciplinary team.

You will assist with daily living activities, grooming, mobility, and safety, while supporting participants during appointments and daily routines. Active CNA with at least 1 year of experience working with frail elderly is required.

Qualifications

• Active CNA in the state where PACE Organization is located is required.
• Minimum of 1 year of experience with frail or elderly populations; if less, complete training provided by PACE on hire.
• Experience in long-term care or geriatric programs preferred.

Responsibilities

• Participate in interdisciplinary team assessments and report changes in participants’ condition.
• Assist with activities of daily living and mobility, including toileting, grooming, bathing, eating, and walking.
• Provide nursing care as assigned safely per policies of the PACE Organization.
• Ensure participant safety, identify hazards, and take corrective actions.
• Provide behavioral supervision and reassurance as needed.
• Assist with meals, recreation, and housekeeping tasks.
• Monitor signs of physical, emotional, or behavioral changes and report to staff.
• Follow infection control and safety procedures; handle hazardous materials and PPE correctly.
• Escort participants to medical and other appointments as needed; assist with transportation loading/unloading.
