Job summary

Fallon Health seeks a Health Aide / CNA to support participants at the PACE center and in homes in Webster, MA. The role includes assisting with daily living activities, contributing to care plans, and collaborating with an Interdisciplinary Team to ensure continuous, high-quality care.

Responsibilities cover vital signs, transfers, grooming, meal support, documentation in EMR, and maintaining a clean, safe environment.

Qualifications

• Completion of an approved Home Health Aide or CNA training program is highly desirable.
• Certification as a Home Health Aide or Nursing Assistant is desired.
• Reliable transportation required.
• One year of experience with frail and/or older adults, preferably in a PACE program.
• If under one year, Summit Eldercare training must be completed prior to independent work.
• Must complete a standardized set of Summit Eldercare competencies annually.

Responsibilities

• Assist with activities of daily living for participants at the PACE center and in their homes.
• Contribute to development of care plans through interaction with the Interdisciplinary Team.
• Collaborate with the Interdisciplinary Team to ensure appropriate and continuous care.
• Carry out non-skilled treatments including vital signs, transfers, toileting, bathing and dressing at the PACE Center and in the community.
• Assist supervisors and staff with individual and group programs, planning, motivation and evaluation of program effectiveness.
• Assist registered therapists with treatments and activities ordered by the therapist.
• Maintain a clean and safe environment in the PACE center and participants’ homes.
• Assist with meal and snack preparation, serving, feeding and clean-up.
• Use safe techniques in all interactions with participants.
• Provide accurate and timely documentation in the EMR and other systems.
• Participate in daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines.
• Participate in distribution of work for health aides to ensure timely care and site task completion.
• Participate in regular team meetings for health aides.
• Perform all duties in accordance with FH and Summit Eldercare policies and procedures.
