Job summary

Linden SNF Healthcare, LLC is seeking a Nursing Assistant to provide routine daily nursing care and services to residents in accordance with care plans and supervision. You will perform essential duties including identifying residents, recording observations, reporting changes, maintaining confidentiality, and assisting with daily activities and transport.

Qualifications include a minimum of a 10th grade education and a valid CNA license. CPR is preferred.

Qualifications

• Must possess, as a minimum, an 10th grade education.
• Language Skills: ability to read, analyze, and interpret general business materials.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Mathematical Skills: apply fractions, percentages, ratios and proportions.
• Reasoning Ability: solve practical problems with concrete variables.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Identify residents before treatments or meals using wristband/photo card.
• Record entries on flow sheets, notes, charts in an informative manner.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse promptly.
• Maintain confidentiality of resident care information and report unauthorized disclosures.
• Assist residents with daily functions and meals; ensure rooms are ready for arrivals.
• Transport residents between rooms and assisting with transfers as needed.
• Provide a warm, calm environment and participate in nursing reports.
