Job summary

Willowbrook SNF Healthcare, LLC (Willowbrook Post Acute) seeks a Certified Nursing Assistant to provide routine nursing care in accordance with each resident's care plan. You will identify residents, assist with daily living activities, document care accurately, and support a warm, safe environment under supervision.

Qualifications

• Must possess, as a minimum, a 10th grade education.
• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• CPR certification preferred.

Responsibilities

• Identify residents before administering treatments or meals using wristbands or photo IDs.
• Report changes in residents' condition to the Nurse Supervisor/Charge Nurse.
• Record entries on charts and flow sheets accurately and promptly.
• Maintain confidentiality of resident health information.
• Assist residents with daily living activities and mobility as trained.
• Prepare residents for medical tests and transport as needed.
• Ensure rooms are prepared for new or transferring residents and greet them appropriately.
