Job summary

Sacramento Post Acute / Oleander Holdings, LLC seeks a dedicated Certified Nursing Assistant to deliver routine daily nursing care in accordance with each resident’s care plan. You will greet residents, assist with daily activities, and help maintain a safe, comfortable environment across the unit.

The role requires a valid CNA license in California, 10th grade education minimum, and CPR is preferred. You will work under the Nurse Supervisor/Charge Nurse and collaborate with the nursing team to

Qualifications

• Must possess, as a minimum, a 10th grade education.
• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• CPR certification is preferred.

Responsibilities

• Provide routine daily nursing care and services as per assessment and care plan.
• Identify residents before treatments or meals using wristband/photo card.
• Report changes in residents' conditions to Nurse Supervisor/Charge Nurse.
• Record entries on flow sheets, notes, and charts accurately and descriptively.
• Maintain confidentiality of resident information and report any breaches.
• Assist residents with daily functions and personal care needs as instructed.
• Observe for pressure areas, skin breakdowns, and report as needed.
• Support a calm, warm, and supportive environment for residents.
