Job summary

PACS is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care in alignment with the resident’s care plan. You will identify residents, document care, and support physicians and nurses in a fast-paced post-acute setting.

Responsibilities include monitoring residents, assisting with meals and activities, and ensuring a safe environment. CPR is preferred; 10th grade education minimum; strong attention to detail required.

Qualifications

• Education and/or Experience must possess, as a minimum, an 10th grade education.
• Language Skills: read, analyze, and interpret general business materials.
• Mathematical Skills: apply fractions, percentages, ratios and proportions.
• Reasoning Ability: solve problems with concrete variables and varied instructions.
• Certificates, Licenses, Registrations: must be a licensed Certified Nursing Assistant per state law; CPR preferred.

Responsibilities

• Provide routine daily nursing care and services per care plan.
• Identify residents with wristband/photo before treatments and meals.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Record entries on flow sheets, notes, and charts clearly and informatively.
• Protect resident information; report any unauthorized access or disclosure.
• Prepare residents for exams and assist with transfers or positioning.
• Assist with daily activities, meals, hygiene, and comfort measures.
• Keep rooms tidy, beds made, and supplies ready for resident needs.
• Observe skin integrity; report decubitus risk and injuries from unknown sources.
• Participate in nursing reports and follow policies on exposure to blood and body fluids.
