Job summary

HoustonTransitionalCare is seeking a Certified Nursing Assistant to provide routine daily nursing care per the resident care plan. You will identify residents, document care activities, and report changes to the Nurse Supervisor.

The role requires maintaining confidentiality and assisting with daily activities, tests, and safe movement of residents. Responsibilities include responding to calls, documenting care, and upholding facility policies.

Qualifications

• Must possess, as a minimum, a 10th grade education.
• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• Ability to read, analyze, and interpret general business materials and regulations.
• Ability to write reports, business correspondence, and procedures manuals.
• Ability to present information and respond to questions from managers and employees.
• Mathematical skills: fractions, percentages, ratios, and proportions.
• Reasoning abilities: solve practical problems with limited standardization.
• Ability to interpret various instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Provide routine daily nursing care and services per resident care plan.
• Identify residents before treatments using wristbands or photo IDs.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Record entries on flow sheets, notes, and charts accurately.
• Maintain confidentiality of resident care information and report any disclosures.
• Assist residents with daily activities including bathing, dressing, and meals.
• Prepare residents for medical tests (lab work, x-ray, therapy, etc.).
• Assist with lifting, turning, moving, and positioning residents.
• Check residents for skin breakdown and report any concerns.
• Respond to resident calls promptly and greet residents professionally.
