Job summary

North Houston Transitional Care is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care and support per residents’ care plans. You will identify residents, record accurate notes, and report changes to the Nurse Supervisor, while maintaining confidentiality and safety.

Responsibilities include assisting residents with daily activities, preparing them for tests, ensuring rooms are ready, and helping with transfers and mobility.

Qualifications

• Must possess, as a minimum, an 10th grade education.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
• Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
• CPR Preferred

Responsibilities

• Identify residents before administering treatments, serving meals, etc.
• Record entries on flow sheets, notes, charts, etc., in an informative manner.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of resident care information and report disclosures.
• Assist residents with daily functions (dental care, bathing, dressing, etc.).
• Prepare residents for medical tests and accompany them as needed.
• Keep resident rooms ready (beds, name tags, etc.).
• Greet and orient residents and introduce them to roommates and staff.
• Assist with mobility and transfers, and transport residents as needed.
• Follow established policies and procedures and work with other staff to ensure quality care.
• Observe skin integrity and report pressure areas to prevent ulcers.
• Report injuries and observe residents for disorientation or changes in condition.
