Qualifications

• Completion of state approved Nursing Assistant Program.
• Current CNA certification/listing in the state registry.
• Experience in skilled nursing or assisted living preferred.

Responsibilities

• Provide direct routine care per Plan of Care and communicate resident needs to the care team.
• Take and record vital signs and monitor for abnormal changes.
• Assist with bathing, grooming, dressing, toileting, feeding, and mobility.
• Support residents' rights and dignity while documenting care accurately.
• Assist with dining and monitoring intake; promote safety and Comfort.
• Collaborate in care conferences and ensure HIPAA-compliant record-keeping.
