Job summary

Hospice of the Midwest in Woodbury, MN is seeking a compassionate CNA to join our hospice team. You will provide 1:1 care, support patients and families, and work within a supportive environment focused on dignity and comfort.

Schedule is Mon-Fri, 8:00 AM - 5:00 PM with opportunities for advancement as the organization grows.

Qualifications

• Meet state training requirements.
• Minimum six months experience including personal care.
• Must be 18+ years old.
• Complete 75 hours of classroom and supervised training, 16 hours before practical training.
• Maintain CPR certification where required.
• Read and follow written instructions; document care.
• Understand hospice philosophy and provide terminally ill care.
• Capable of working with minimal supervision.

Responsibilities

• Provide personal care: bathing, dressing, toileting, mobility, repositioning.
• Monitor and report changes in patient condition.
• Assist with ambulation, transfers, ROM and safe equipment use.
• Support patients with self-administered medications per state rules.
• Offer companionship and emotional support to patients and families.
• Perform light housekeeping: linens, meals, space cleanliness.
• Maintain accurate and timely documentation.
• Collaborate with hospice interdisciplinary team.

Responsibilities

• Personal Care: Help with bathing, dressing, toileting, mobility, and repositioning

• Mobility: Assist with ambulation, transfers, ROM exercises and safe equipment use
• Medication Assistance: Help with self-administered medications per state regulations
• Emotional Support: Offer companionship and comfort to patients and families
• Light Housekeeping: Change linens, assist with meals, and maintain a clean space
• Documentation: Maintain accurate and timely documentation
• Team Participation: Collaborate with hospice interdisciplinary team

Qualifications

• Meet the training requirements of the state of practice
• Minimum of six months experience including providing personal care
• Must be 18+ years of age
• Complete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical training
• Possess and maintain current CPR certification in states where required.
• Ability to read and follow written instructions and document care given
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill
• Ability to work with little direct supervision
• Ability to handle death/dying
• Strong oral and written communication skills
• Good Organizational Skills
