Coverage Area: Monroeville/Irwin/Murrysville/New Kensington; travel throughout territory may be required

Schedule: Full-Time, Monday-Friday, 8AM-4:30PM

Responsibilities
  • Perform personal care needs, promptly attending to patient requests and reporting any inability to perform tasks to the Case Manager.
  • Communicate positively with patients and families, maintaining confidentiality and ethical standards.
  • Ensure patient safety using equipment properly, including foot stools, side rails, and oxygen.
  • Provide personal care such as baths, back rubs, oral hygiene, shampoos, and linen changes.
  • Assist with dressing, undressing, and meal preparation, including shopping and feeding as needed.
  • Monitor and record vital signs, including temperature, pulse, respiration, and blood pressure, when ordered.
  • Observe and report on patient's skin condition to prevent tissue breakdown.
  • Assist with ambulation, exercises, and positioning as instructed.
  • Support medication reminders and help with light laundry and household tasks.
  • Participate in quality improvement activities and adhere to documentation standards.
  • Respect patient and family privacy and environment.
Qualifications
  • Meet state training requirements; completed 75 hours of classroom and supervised practical training.
  • Minimum six months of experience providing personal care.
  • At least 18 years old, with the ability to read, follow instructions, and document care.
  • Understand hospice philosophy and provide compassionate, specialized care to terminally ill patients.
  • Current CPR certification; able to work independently and maintain professionalism.
  • Good communication, organizational skills, and tactfulness.
  • Successfully complete a probationary period demonstrating competence.
