Overview

Our Hospice Aide/CNAs are the heart of our organization, supporting patients through personal care services to meet their needs. Schedule: Full-Time, Monday-Friday, 8AM-4:30PM. Location: 260 Alpha Drive, Pittsburgh, PA 15238. Coverage includes Monroeville, Irwin, Murrysville, New Kensington, with travel required throughout the territory.

Responsibilities

1. Perform personal care needs and attend to patient requests promptly, reporting to the Case Manager if unable to perform a task.
2. Provide positive communication and maintain confidentiality with patients and families.
3. Ensure safety using equipment properly.
4. Assist with bathing, grooming, dressing, and other personal care activities.
5. Plan and prepare nutritious meals and assist with feeding as needed.
6. Monitor and record vital signs and observe patient skin condition.
7. Report patient condition changes to the Case Manager.
8. Assist with ambulation, exercises, and transfers.
9. Administer reminders for self-administered medications, and assist with light laundry and bedpans.
10. Participate in quality improvement activities and uphold professional standards.

Qualifications

1. Meet state training requirements with at least 75 hours of training and practical experience.
2. Minimum six months experience providing personal care.
3. At least 18 years old, with the ability to read, follow instructions, and document care.
4. Understand hospice philosophy and provide specialized care for terminally ill patients.
5. Possess current CPR certification and demonstrate tact, patience, and good hygiene.
6. Successfully complete a probationary period demonstrating competence.
7. Excellent communication and organizational skills.
