Holton Manor in Elkhorn, WI is seeking a Certified Nursing Assistant (CNA) to provide patient/resident care under licensed nurse supervision. You will perform essential tasks to ensure timely, safe care and communicate with patients, families, and the health team.

Qualifications

• Must be in good standing on the state Nursing Assistant registry or equivalent.
• Ability to meet physical and job-related requirements.
• Must be English literate with excellent written and verbal communication skills.
• Preferred: hospital, long-term care, or skilled nursing experience.
• CPR certification is preferred.

Responsibilities

• Performs initial and ongoing data collection on patients/residents under RN/LPN supervision.
• Implements care plans developed by the RN for patient needs.
• Provides direct patient care and reports changes to RN/LPN.
• Demonstrates time management and prioritization of tasks.
• Documents care in accordance with policies and scope of practice.
• Nutritional care and supporting residents’ dietary needs.
• Communicates effectively with residents and families.
• Completes all other duties as assigned.

Key Responsibilities:

• Performs initial and ongoing data collection on all patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
• Implementsplan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural,ethnicand religious/spiritual needs.
• Provides direct patient/resident care andreportscondition and changes to RN or LPN.
• Demonstrates time management and prioritization of tasks.
• Document results of patient/resident care within established policies,proceduresand scope of practice.
• Nutritional care
• Effective communication with residents and family
• Completes all other duties as assigned.

Qualifications:

• Required:
