Job summary

Polaris Rehabilitation & Care Center is seeking compassionate CNAs to provide direct, hands-on care to residents and support daily living activities. You will collaborate with nurses and interdisciplinary team members to ensure safe, respectful care.

Qualifications

• Current, active CNA certification in the State of Wyoming.
• Certification must be in good standing with no disqualifying findings on the Nurse Aide Registry.
• Experience in skilled nursing, rehabilitation, long-term care, or related healthcare environment is preferred but not required.
• Strong communication and interpersonal skills.
• Ability to follow individualized resident care plans and nursing direction.

Responsibilities

• Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, feeding, and personal hygiene.
• Obtain and record vital signs (temperature, pulse, respirations, blood pressure).
• Observe resident condition changes and report to supervising nurse.
• Assist with transfers, ambulation, repositioning, and mobility equipment per care plans.
• Respond to call lights and resident requests promptly.
• Assist with meal service and feeding when required.
• Provide routine skin care, incontinence care, oral care, and other personal care services.
• Document care provided per facility procedures.
• Follow care plans and communicate concerns to licensed nursing staff.
