Job summary

Polaris Rehabilitation & Care Center is seeking compassionate CNAs to join our nursing team in Cheyenne, WY. You will provide direct, hands-on care, assist with daily living activities, and support the interdisciplinary team to ensure safe, respectful, and individualized resident care.

Responsibilities include ADLs, vital signs, transfers, mobility, meal support, and accurate documentation. We value teamwork, ongoing development, and a path toward career advancement within the facility.

Qualifications

  • Wyoming CNA certification in good standing
  • Experience in skilled nursing or related care preferred but not required
  • Strong communication and ability to follow care plans

Responsibilities

  • Assist residents with ADLs including bathing, dressing, grooming, toileting, feeding
  • Record vital signs accurately and report concerns to supervising nurse
  • Observe and report changes in condition or behavior promptly
  • Assist with transfers, ambulation, and repositioning per care plans
  • Respond promptly to call lights and resident requests
  • Provide feeding assistance and assist with meal service as needed
  • Deliver routine skin, incontinence, oral, and other CNA-level care
  • Document care in facility systems following procedures
  • Follow resident care plans and communicate changes to licensed staff
  • Maintain clean, safe care areas and equipment
  • Follow infection control and resident safety procedures
  • Respect resident privacy, dignity, and rights
  • Communicate respectfully with residents, families, and team members
  • Comply with all facility policies and Wyoming/federal regulations

Position Overview

Polaris Rehabilitation & Care Center is seeking compassionate, dependable, and resident-focused Certified Nursing Assistants (CNAs) to join our nursing team.

As a CNA, you will provide direct, hands-on care to residents while assisting with activities of daily living, mobility, personal care, and other essential needs. You will work closely with nurses and other members of the interdisciplinary team to help ensure each resident receives safe, respectful, and individualized care.

Our CNAs are an essential part of our clinical team. This position is ideal for someone who enjoys building relationships with residents, working as part of a team, and making a meaningful difference during every shift.

Key Responsibilities
  • Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and personal hygiene
  • Obtain and accurately record vital signs, including temperature, pulse, respirations, and blood pressure
  • Observe residents for changes in physical condition, behavior, mood, mobility, appetite, skin condition, or overall well-being and promptly report concerns to the supervising nurse
  • Assist residents with transfers, ambulation, repositioning, and mobility equipment in accordance with individualized care plans and established safety procedures
  • Respond promptly and professionally to call lights and resident requests
  • Assist with meal service and provide feeding assistance when required
  • Provide routine skin care, incontinence care, oral care, and other personal care services within the CNA scope of practice
  • Accurately document care and services provided in accordance with facility procedures
  • Follow resident-specific care plans and communicate concerns or changes to licensed nursing staff
  • Maintain resident rooms and care areas in a clean, safe, and organized condition
  • Follow infection prevention, fall prevention, transfer, skin integrity, and resident safety procedures
  • Protect resident privacy, dignity, independence, preferences, and rights
  • Communicate respectfully with residents, families, visitors, nurses, and interdisciplinary team members
  • Follow all facility policies and applicable federal and Wyoming requirements
Qualifications
  • Current, active Certified Nursing Assistant (CNA) certification in the State of Wyoming
  • Certification must be in good standing with no disqualifying findings on the applicable Nurse Aide Registry
  • Previous experience in skilled nursing, rehabilitation, long-term care, assisted living, or another healthcare environment preferred but not required
  • Strong communication and interpersonal skills
  • Ability to follow individualized resident care plans and nursing direction
  • Dependable and able to work effectively as part of a clinical team
  • Strong observation skills and ability to recognize and report changes in resident condition
  • Ability to prioritize responsibilities in a fast-paced healthcare environment
  • Commitment to providing compassionate, respectful, and resident-centered care
  • Ability to perform the essential functions of the position with or without reasonable accommodation
