Job summary

Lutheran Social Services of WI & Upper Michigan is hiring a full-time Care Coordinator for Ozaukee County to support children and families in the CLTS Waiver Program. You will assess needs, develop family-centered plans, and connect families with services while collaborating with partners.

A hybrid schedule and standard benefits are offered. You will serve as the main contact for families, coordinate resources, and document progress with trauma-informed, person-centered support.

Qualifications

• Bachelor's degree in Social Work or a related Human Services field.
• At least one year of experience working with children and/or youth with disabilities or an equivalent combination of education and experience.
• Valid driver's license and reliable transportation.
• Strong communication, organization, and documentation skills.

Lutheran Social Services of WI & Upper Michigan is seeking a full-time Care Coordinator to support children and families participating in the Children's Long-Term Support (CLTS) Waiver Program in Ozaukee County. In this role, you will help families access services and resources that enable children with developmental, physical, or severe emotional disabilities to thrive safely at home and in their communities.
